OFFICIAL RULES FOR
SALONPAS Japan Holiday Sweepstakes 2008

  1. No purchase necessary.
  2. The SALONPAS Japan Holiday Sweepstakes 2008 will begin on October 1, 2008, and end at midnight on November 30, 2008.
  3. There are two (2) ways to enter the contest :
    1. Provide your name, address, telephone number and age along with 3 barcodes from the packs of Salonpas (40 Sheets) and mail to : Hisamitsu America, Inc., 3528 Torrance Blvd., Suite 112, Torrance, CA 90503 ; or
    2. Request an alternative entry form by sending a stamped self addressed envelop to: Hisamitsu America, Inc., 3528 Torrance Blvd., Suite 112, Torrance, CA 90503, and send the completed entry form back to Hisamitsu America Inc.
  4. Entries must be received by midnight on November 30, 2008 to be deemed eligible.
  5. The contest is open to U.S. citizens and legal U.S. residents who are at least 21 years old and currently reside in the state of California.
  6. Singapore Airlines, Narita Airport Limousine, Hotel Pacific Tokyo (SHINAGAWA) and Salonpas/Hisamitsu America are not responsible for lost, stolen, mutilated, miss delivered, postage due, illegible or untimely entries.
  7. Entries become the property of Salonpas/Hisamitsu America, Inc.
  8. Employees of Salonpas/Hisamitsu America, Inc., its licensee, its corporate parent, Hisamitsu Pharmaceutical Co., Inc., their affiliated entities, affiliated advertising agencies, Singapore Airlines, Narita Airport Limousine, and Hotel Pacific Tokyo (SHINAGAWA) and the member of their immediate families are ineligible to participate and win.
  9. One (1) Grand Prize will be awarded, which includes round trip Economy class air transportation for two (2) on Singapore Airlines from Los Angeles International Airport to Tokyo, Japan, transfers between Narita Airport & Hotel Pacific Tokyo (SHINAGAWA) by Airport Limousine Coach for two (2), and 4 nights/5 days one room hotel accommodations (double occupancy) at Hotel Pacific Tokyo (SHINAGAWA) inclusive of hotel service charge and government room tax.
  10. One (1) 1st Prize will be awarded for $100.00 Gas Card.
  11. Two (2) 2nd Prize will be awarded for $50.00 Gas Cards.
  12. Five (5) 3rd Prize will be awarded for $20.00 Gas Cards.
  13. Ten (10) 4th Prize will be awarded for SALONPAS Trial Set (Retail Value $30.00 per set).
  14. Winner is solely responsible for all other expenses, including but not limited to food and other necessary travel related expense.
  15. Travel reservations made through Salonpas / Hisamitsu America, Inc.
  16. Travel based on availability, is valid Monday-Thursday only, and must be completed within three (3) months of certificate issue date.
  17. All blackout periods apply.
  18. After winner has been ticketed, the winner is unable to change ticketed departure date from Los Angeles International Airport for going to Tokyo, Japan by Singapore Airline.
  19. You may change return date at Tokyo Japan, directly contact with Singapore Airline for paying additional $100 changing fee per person (returning date between Friday and Sunday will be charged additional $25 per person). Singapore Airline contact information at Tokyo Japan will be provided.
  20. The winner has total responsibility to contact with Hotel Pacific Tokyo (SHINAGAWA) to change or extend check out date and paying additional room charge. Salonpas / Hisamitsu America, Inc does not have any responsibility for the winner’s changing fee, additional room charge fee, or any other necessary fee for the winner to change their ticketed itinerary.
  21. The winner has total responsibility to contact with Airport Limousine to change ticketed return date from Hotel Pacific Tokyo (SHINAGAWA) to Narita (Tokyo) International Airport. The winner may ask Hotel Pacific Tokyo (SHINAGAWA)’s concierge or front desk for necessary arrangement.
  22. The approximate value of the Grand prize is $2,310.00 for two people.
  23. The prize is not transferable, and cannot be substituted or redeemed for cash by the winner.
  24. All winners will be selected by Salonpas / Hisamitsu America, Inc. in a random drawing from among all eligible entries received.
  25. The prize drawing will be conducted on or about Friday, December 5th, 2008.
  26. Decisions by Salonpas / Hisamitsu America, Inc. with respect to the contest are final.
  27. The winner will be officially notified on or about Monday, December 8th, 2008 via phone, certified mail or UPS.
  28. If the winner cannot be reached within 3 days or is determined to be ineligible, a replacement winner will be randomly selected from among all eligible entries received about Friday, December 12th, 2008 and notified via phone, certified mail or UPS about Wednesday, December 17th, 2008.
  29. Winner need not purchase the Salonpas or be present to win.
  30. Payments of all applicable federal, state and local taxes relative to the value of the prize are the sole responsibility of the winner. Winner may be required to sign an IRS Form W-9 or the equivalent.
  31. The winner will be required to sign and return an affidavit of eligibility and release of liability prior to being awarded the prize. By participating in the contest, the winner agrees to have his or her name, voice, and/or likeness used in any advertising or broadcasting material relating to this contest without additional financial or other compensation, and to sign a publicity release prior to acceptance of the prize.
  32. Contest sponsors are not in any way liable for damage, loss or injury resulting from the awarding or use of the prize. Winner assumes all liability for injuries resulting, caused, or claimed to be caused by participation in this contest, or from the use of the prize awarded, including any travel related thereto.
  33. Contest winner (and traveling companion) will be required to sign a liability release prior to the acceptance of the prize.
  34. Pursuant to the liability release, the winner (and traveling companion) will agree to hold Salonpas / Hisamitsu America, Inc. Singapore Airlines, Airport Limousine, and Hotel Pacific Tokyo (SHINAGAWA), its corporate licensee, its corporate parent, Hisamitsu Pharmaceutical Co., Inc., their affiliated companies and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the use or possession of the prize or participation in the contest.
  35. Salonpas / Hisamitsu America, Inc., which is not affiliated with Singapore Airlines, Airport Limousine, Hotel Pacific Tokyo (SHINAGAWA), reserves the right to amend these contest rules as may be necessary, and to substitute a prize or prizes of equivalent value, which will become effective upon announcement.
  36. The contest is void where prohibited.
  37. Failure to comply with the contest rules may result in a contestant’s disqualification, at the sole discretion of Salonpas / Hisamitsu America, Inc., Singapore Airlines, Airport Limousine, and Hotel Pacific Tokyo (SHINAGAWA).
  38. Salonpas / Hisamitsu America, Inc. is not responsible for typographical or other errors in the printing, the offering or the administration of the contest or in the announcement of a prize.
